Policies & Fees


Policies:

  • Our initial appointment will be for taking your history, evaluating your condition, and beginning treatment. This usually requires about an hour. Subsequent appointments last about a half hour and consist of checking in and treatment.
  • Frequency of appointments is based on an ongoing evaluation of your condition and needs.
    Full payment is expected for appointments not cancelled by the scheduled day. Emergency situations necessitating last minute cancellation will certainly be considered for exception.
  • Arriving late for appointments will often necessitate shortening the time we have to work together.
  • Please avoid wearing perfume, cologne, or any strong scents as many people are allergic to these substances.


Insurance:

  • Dr. Gottlieb is enrolled as a provider with Blue Cross/Blue Shield of Massachusetts. For other insurance plans he will provide receipts and paperwork needed for you to submit for reimbursement.


Fees:

  • Initial one hour visit: $200
  • Half hour follow up visits: $100
  • If the work and time needed for follow up visits routinely exceeds a half hour then a higher rate for these visits will reflect the additional time required. This will be discussed and agreed upon prior to the appointments involved.
  • Payment over time is available if needed. Sliding scale will be considered for those with financial constraints. This must be discussed and agreed upon prior to treatment.


Payment:

  • Payment is expected at the time of treatment unless other arrangements are made in advance. Cash, check, Visa, or Mastercard are accepted.
